Description
This project deals with development of a standalone desktop based database application for a LUNA (Latinos Understanding the Need for Adherence) study to aid the Chula Vista clinic in managing the patient and participant information in a more meaningful manner. This application will also be of assistance to the administrative workforce. The application is a collection of tables, queries, macros and reports that helps clinic personnel get information that's just a click away. It holds various patient information including statistical data about the patients, e.g. blood pressure, heart beat rates etc. One can also search for the information about a patient from thousands of records and can view at any time the status of a participant's progress in the study. There are four specifications of this application: the general study visits of participants, the special intervention group (a study to modify multiple cardiovascular risk factors that can be measured by sonography), the re-admin form (a phase following intervention, where participants will undergo routing care and health surveys re-administered) and the observational group chart abstraction. The issue of the study is adherence to intervention strategies for prevention of conditions like heart failure, diabetes, hypertension and asthma.
